Easterseals MORC is seeking a Case Management Coordinator - Family Services to support minors and families in Macomb. The role involves evaluation, planning, and collaboration with clinical teams to connect clients with community resources and services.

A CMHP license and related experience are preferred, with a commitment to compassionate care and strong client advocacy within a multidisciplinary team.

Qualifications

• Must be a CMHP per Medicaid Provider Manual Guidelines
• Bachelor's degree in Psychology or Social Work and experience with minors and families
• Valid driver's license required

Responsabilités

• Engage individuals in a welcoming, hopeful, empathic manner
• Assess and monitor needs, develop strength-based plans, and coordinate services
• Complete referral forms and document communication with community partners
