Job summary

Easterseals MORC is seeking a Case Management Coordinator for Community Outpatient services in Auburn Hills, MI. This role focuses on engaging individuals, assessing needs, and coordinating services to support recovery and community integration.

Qualifications

• Bachelor's degree requirements in Human Services, Psychology or Social Work or related field.
• LLMSW/LMSW/LLPC/LPC/TLLP/LLP credentials with Michigan registration are acceptable.
• Must hold CMHP status in accordance with Medicaid Provider Manual Guidelines.

Responsibilities

• Engage individuals in a welcoming, hopeful, empathetic manner regardless of disability or recovery phase.
• Screen for co-occurring disorders and identify diagnostic criteria for substance use issues.
• Assess and evaluate needs; maintain caseload contact to develop goals and person-centered plans.
• Complete referral forms and documentation to obtain community based services and coordinate with SA/MH partners.
