Job summary

The Salvation Army, A Georgia Corporation in San Antonio, Texas, seeks a Case Manager to deliver long‑term, structured services for an assigned client caseload within an established life management program. You will assess needs, develop program plans, mentor clients, and ensure compliance with funding requirements.

You will interview clients, document eligibility and progress, maintain HMIS records, and advocate to access necessary services to support client recovery and independence.

Qualifications

• Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field.
• Three years progressively responsible experience providing direct case management social services, including needs assessment and developing individual, comprehensive, long-term action plans for recovery.
• Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Responsibilities

• Greets and interviews clients to determine eligibility for assistance and records disposition and pertinent information.
• Establishes long-term program goals for eligible clients and counsels on modifying negative behaviors within the program.
• Prepares and maintains case records and logs, and enters information into HMIS.
