Qualifications

  • Discretion and effective communication with staff, volunteers, clientele and the public.

Responsibilities

  • Provides ongoing case coordination; conducts intake interviews and assesses needs; develops service plans.
  • Advocates, supports, and connects participants to social services and employment opportunities; makes referrals as needed.
  • Maintains case files and required documentation per agency standards; handles clerical duties and CMIS/RIS data systems.
  • Performs home visits as needed to address health, safety, and job preparedness; arranges transportation to services.
  • Maintains relationships with staff and external resources; informs supervisor of activities to ensure contract compliance.
  • Represents the refugee program and the Agency in the community; responds to inquiries.

Description
POSITION SUMMARY:

Assumes primary responsibility for direct case coordination services for participant self-sufficiency and self-reliance; ensures compliance with all relevant contractual requirements, as well as agency policy and procedures; as assigned, directs the work of staff/volunteers in provision of participant services. This position is limited duration and intended to support short-term needs, with the duration subject to change based on organizational needs.

ESSENTIAL FUNCTIONS:

1. Provides on-going case coordination; conducts intake interviews and assesses needs of participants; develops service plans.

2. Provides advocacy, support, and connection to social services agencies and to appropriate employment, advising and assisting participants or making appropriate referrals.

3. Establishes and maintains case files; performs clerical duties associated with case coordination, including correspondence and reports. Prepares and maintains required documentation and requirements of the agency’s CMIS and State Refugee Office’s RIS data base systems.

4. As appropriate, conducts home visits to address health, safety, and job preparedness issues; transports participants, or directs such activity, as necessary to ensure provision of services.

5. Maintains working relationships with staff, other diocesan and external resources to coordinate participant services and provide advocacy as needed; advises Supervisor of activities to ensure that services offered are in keeping with the contract requirements.

6. Represents the refugee program and the Agency in the community; responds to inquiries.

OTHER RESPONSIBILITIES:
  • Participates in EOS and supports Agency compliance with relevant accreditation standards.
  • Participates in outreach and development activities as needed.
  • Participates in program, agency, diocese and stakeholder meetings and training as required.
  • Performs other duties as required, in collaboration with Supervisor and other program staff.
Requirements
EDUCATION, EXPERIENCE AND SKILLS REQUIRED:
  • Graduation from high school or equivalent.
  • Experience with diverse cultures.
  • Continuous possession of a valid driver’s license; continuous status as an Approved Driver by the Catholic Diocese of Charlotte; continuous insurance coverage meeting requirements of the Catholic Diocese of Charlotte on any personal vehicles used as part of employment.
  • Fluency in foreign language of program participants preferred but not required.
  • Skills required: Work with discretion, and communicate effectively with staff, volunteers, clientele and the public in a culturally diverse working environment; prepare and maintain records according to contract and agency standards; communicate fluently, orally and in writing, in English and specific native language as applicable, to perform the essential functions this position.
WORKING ENVIRONMENT:

Responsibilities of the Case Aide may involve travel and work beyond the regularly scheduled workday, and will involve direct interactions with program participants, including in the participants’ home, agency/personal vehicle or various public settings.
