Job summary

Deaconess Hospice, part of the LHC Group and within the Optum family, is hiring a Home and Community Based Services Aide to support individuals at home. Responsibilities include personal care, daily living activities, and accompanying clients to appointments.

The role emphasizes safety, confidentiality, and respectful care in diverse home environments. Requirements include a valid driver's license and reliable transportation; CPR is preferred or can be completed within 90 days.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Preferred: 6+ months of home care experience.
• Preferred: Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
