Job summary

UnitedHealth Group, through Deaconess Hospice (LHC Group, Optum family), seeks a Hospice Aide to provide in-home personal care and health-related services to patients and families. The role focuses on maintaining comfort and well-being while supporting daily living activities.

The Hospice Aide will document client observations, travel safely to patient homes, and collaborate with the care team. A valid driver’s license and competency evaluation are required, with preferred CPR and 6+ months home

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation.
• Successful completion of a competency evaluation and one of the following: previous work experience as a nurse's aide or home health aide or completion of approved home health aide course.

Responsibilities

• Provide in-home personal care and designated health-related services to maintain the patient’s physical and emotional well-being.
• May assist with activities of daily living including bathing, hair care, denture care, nail care, skin care, and transfers.
• Report observations to the agency director or accounts manager.
• Drive safely and comply with traffic regulations during business travel.
• Assist with household tasks essential to clients’ personal care.
