Job summary

Hartford HealthCare is seeking a Homemaker Caregiver to support Independence at Home patients across Connecticut. You will provide essential in-home care, assist with daily activities, meals, and personal care, and coordinate with nurses and coordinators to ensure a safe, dignified plan of care.

Ideal candidates have a driver's license and CNA background, with 1 year of care experience and ability to travel within 30 minutes of home.

Qualifications

• Driver's license and auto insurance required.
• 1 year of personal care experience at a facility or agency.
• 2 years of continuous work experience required.
• New Grad CNAs are welcome.
• Caregivers must be comfortable driving within 30 minutes of residence.

Responsibilities

• Provide personal hygiene and grooming care to clients.
• Assist with toileting and ambulation.
• Prepare meals for clients' nutritional needs.
• Report changes in clients' condition to nurse, supervisor, or client care coordinator.
• Perform routine housekeeping and home management duties.
• Record each client's care on care plan and submit in a timely manner.
• Assist with providing emotional support for clients and families.
• Assist with routine therapy exercises.
• Participate in case conferences as requested.
• Attend all required in-service education sessions.
• Use computer software and smartphone for required education.
• Represent the company with a high level of customer service.
• Think critically without direct supervision and be able to expand when appropriate.

Shift Detail: Hourly cases range anywhere from 4- 12 hours at any time in the day, evening or night. Shifts will take place within 30 minutes of where you reside.

The Homemaker Caregiver Position Provides
