Job summary

Responsibilities include coaching, documenting progress, creating an Individual Enhancement Plan, suggesting development activities, and coordinating with service providers to monitor outcomes.

Qualifications

• Minimum BA/BS degree; one year experience in criminal justice, social work, employment counseling, placement, human services, or public administration.
• Excellent oral and written communication skills.
• Proficient with computer and Microsoft Office; familiarity with social media apps.
• Must provide own transportation; DMV check; valid driver’s license and vehicle registration; ambulatory.

Responsibilities

• Maintain case documentation with progress notes and compliance with grant requirements.
• Create an Individual Enhancement Plan (IEP) and perform an interest assessment.
• Suggest appropriate development activity and assist in personal development planning.
• Provide timely guidance to strengthen knowledge and skills toward mentees’ objectives.
• Conduct case management activities, travel to assist clients, prepare case summaries and reports, attend meetings and training sessions, refer to outside agencies.
• Monitor client progress through calls, site visits, and appointments; liaise with project partners delivering services.
• Keep current with job-related rules, statutes, and laws; read professional literature; attend workshops.
• Inform Program Manager of unusual or crisis situations as they arise.
