Job summary

Southwest Key Programs seeks a Career Coach in Port Arthur, TX to assist TANF/SNAP/ABAWD/NCP participants. You will verify eligibility, assess needs, and build employment plans while coordinating supports like childcare and transportation.

You will conduct follow-ups to ensure progress, maintain confidentiality, and document activities in TWIST/WIT/COLTS. Telework is available as needed, with travel to engage communities.

Qualifications

• Bachelor's degree preferred with 1 year case management or 60 credit hours plus 3 years experience.
• Excellent oral and written communication skills and professional appearance.
• Must have own transportation to perform duties; bilingual is a plus.

Responsibilities

• Provide information to TANF, SNAP, ABAWD, and NCP participants and verify eligibility.
• Conduct assessments to determine participant needs and develop employment plans.
• Provide case management and coordinate supportive services like childcare/transportation.
• Follow-up with participants to document activities and progress.
• Monitor progress and take timely action if goals aren’t met.
• Prepare and monitor documentation in TWIST/WIT/COLTS with confidentiality.
• May facilitate group orientations.
