Job summary

Children's National Hospital in Washington, DC is seeking a Clinical Resource Management Associate to support the Care Management team. Under RN supervision, you will coordinate access to services, schedule appointments and ensure safe transitions of care for patients and families.

You will work with families, payers and multidisciplinary teams, performing administrative tasks with strong organizational skills and a commitment to compassionate, high-quality care.

Qualifications

• Minimum education: Associate's Degree.
• Minimum work experience: 2 years in a healthcare or health-related setting.

Responsibilities

• Schedule specialty appointments and outpatient procedures with family preferences.
• Prioritize appointments to prevent discharge delays and coordinate with PCPs.
• Conduct post-discharge follow-up calls and triage patient issues to care team.
• Collaborate with Case Management, Social Work and physicians to meet patient/family needs.
