Job summary

Children's National Hospital is seeking a Clinical Resource Management Associate to support the Care Management team in coordinating safe, coordinated transitions of care for pediatric patients and families.

You will work under the supervision of a Registered Nurse Case Manager, collaborating with patients, families, payers, and multidisciplinary teams to facilitate access to services and promote positive outcomes.

Qualifications

• Associate's Degree is required.
• Minimum 2 years in a healthcare or health-related setting.

Responsibilities

• Schedule specialty appointments and procedures, prioritizing and communicating with families and the medical team.
• Support discharge phone calls, triage issues, and maintain the discharge follow-up database.
• Collaborate with Case Management, Social Work and physicians to meet patient/family needs and coordinate transitions of care.
• Assist in performance improvement efforts to enhance workflow and customer service.
