Job summary

Children's National Hospital is seeking a Clinical Resource Management Associate to support the Care Management team in coordinating safe, coordinated transitions of care for pediatric patients and families.

You will work under the supervision of a Registered Nurse Case Manager, collaborating with patients, families, payers, and multidisciplinary teams to facilitate access to services and promote positive outcomes.

Qualifications

  • Associate's Degree is required.
  • Minimum 2 years in a healthcare or health-related setting.

Responsibilities

  • Schedule specialty appointments and procedures, prioritizing and communicating with families and the medical team.
  • Support discharge phone calls, triage issues, and maintain the discharge follow-up database.
  • Collaborate with Case Management, Social Work and physicians to meet patient/family needs and coordinate transitions of care.
  • Assist in performance improvement efforts to enhance workflow and customer service.

What You’ll Do:

As a Clinical Resource Management Associate, you will play a vital role in supporting the Care Management team and helping ensure safe, coordinated transitions of care for our patients and families. Working under the supervision of a Registered Nurse Case Manager, you will collaborate with patients, families, vendors, payers, community agencies, and multidisciplinary healthcare teams to facilitate access to services, support appropriate utilization of healthcare resources, and promote positive patient outcomes. Through strong organizational skills, attention to detail, and exceptional customer service, you will help coordinate administrative and operational functions that contribute to seamless care delivery across the continuum.

What You'll Need:

Minimum Education: Associate's Degree (Required)

Minimum Work Experience: 2 years Healthcare or health related setting required. (Required)

Functional Accountabilities
  • Patient Scheduling Gather information required to make specialty appointments and outpatient procedures and take family preferences (day of week, location) into consideration when scheduling appointments. Apply knowledge of Children’s, its processes and payer requirements when scheduling appointments. Facilitate scheduling needed appointments (specialist, procedures) and work with the family to schedule PCP (primary care physician) appointment. Prioritize appointments to prevent discharge delays. Communicate scheduled appointments to the family and medical team using accepted processes.
  • Discharge Phone Call Support Work with the Case Manager(s) to prioritize the calls to be made on a daily basis. Use the Phone Call Script to complete post discharge calls and triage care failures/patient issues to appropriate member of the care team. Apply clinical knowledge when conducting follow up phone calls. Maintain database on completed phone calls.
  • Transition of Care Collaborate with Case Management, Social Work and physicians to meet patient/family needs. Utilize Case Management screening tool to gather pertinent information from patient/family. Interact and respond to inquiries from families, payers, external providers, and medical team by answering questions and explain information clearly and thoroughly to facilitate safe transition for patients. Fax discharge summaries for patients who don’t have PCP fax number in Cerner system.
  • Performance Improvement Participate in the identification, implementation and evaluation of processes to improve work flow and customer service.

The disclosed salary range includes the minimum and maximum rates within which Children’s National believes an individual’s base pay rate will fall for this position. It is not typical for an individual to be hired at or near the maximum of the pay range. The exact pay rate for this position will be based on a variety of factors in alignment with the Children’s National compensation philosophy. These factors are legitimate and non-discriminatory including, but not limited to, the current market conditions; organizational needs; the individual’s combination of prior work experience, level of education, knowledge, skills, and other qualifications. Children’s National is committed to providing a fair and competitive total rewards package to each of our employees. This base salary range does not include our comprehensive benefits package or any additional compensation for which this position may be eligible.
