Qualifications

• Active, unrestricted registered clinical license in the state of hire.
• 5+ years of clinical experience.
• Reside within or near the county listed on the job description.
• Driver's License and access to reliable transportation.

Responsibilities

• Engage with patients, families, or caregivers either telephonically or on-site weekly and as needed, and assist in discharge goals.
• Collaborate with care teams with individualized communication and participate in interdisciplinary meetings.
• Review target outcomes and discharge plans with providers and families.
• Perform SNF assessments on patients using CMS criteria at admission and periodically during stay.
• Assess continued appropriateness for SNF setting according to CMS criteria.
• Complete SNF continued stay reviews and update authorizations promptly.
• Review referrals when utilization management is delegated and escalate to licensed physicians as needed.
• Maintain documentation timely and accurately in Coordinate.
• Adhere to policies and protect PHI in compliance with HIPAA and laws.
