Qualifikationen

• 5+ years of clinical experience.
• Reside within or near the county listed on the job description.

• Engage with patients, families, or caregivers weekly telephonically or on-site; collaborate on discharge goals.
• Coordinate with interdisciplinary teams and attend meetings.
• Review discharge plans with providers and families; perform SNF assessments.
• Monitor patient appropriateness for SNF; update authorizations timely.
• Enter accurate documentation and maintain PHI confidentiality.
• Adhere to CMS guidelines, policies, and local regulations.
