Optum Home & Community Care is seeking a licensed clinician to support transitions from hospital to home and SNF settings. You will collaborate with interdisciplinary teams, review discharge plans, and perform CMS-guided SNF assessments to ensure appropriate continued care.

The role blends on-site and home-based work within a 30‑mile radius of the assigned location. Applicants should have 5+ years of clinical experience, an active RN/PT/OT/SLP license, and a valid driver’s license with reliable
