Optum Home & Community Care is seeking a licensed clinician to join our Care Transitions team serving Riverside and surrounding areas.

You will perform SNF assessments, aid discharge planning, coordinate with providers, and ensure CMS criteria are met while maintaining HIPAA compliance. The role requires driving within a 30-mile radius, on-site and home-office hybrid work, and strong documentation skills.

Qualifications

• Active, unrestricted clinical license required in state of hire (RN, PT, OT, or SLP)
• 5+ years of clinical experience
• Residence within or near the listed county with 30-mile radius from home location

Responsabilités

• Perform SNF assessments using CMS criteria on admission and during stay
• Review target outcomes and discharge plans with providers and families
• Manage assigned caseload with efficient time management
• Enter timely, accurate documentation into Coordinate system
• Adhere to HIPAA and local, state, and federal regulations
