Job summary

Qualifications

• Minimum of 3 years of relevant experience in case management or care coordination.
• Case Management Certification (ACM/CCM/ANCC) required within 3 years of hire.
• For BSN-prepared candidates: valid RN license (NC or compact) required.

Responsibilities

• Assess patients for case management needs and discharge planning.
• Develop, implement, and evaluate individualized discharge plans.
• Coordinate services like Home Health, DME, transportation, SNF referrals.
• Monitor census and communicate findings with the care team.
• Educate patients and families on benefits, reimbursements, and regulatory requirements.
• Collaborate with Utilization Management to optimize resource use.
