Job summary

Duke Regional Hospital in Durham, North Carolina, is hiring a Case Manager to coordinate discharge planning and care transitions across the continuum of care. You will work with interdisciplinary teams, patients, families, and community partners to ensure timely assessments and safe transitions.

Minimum qualifications include BSN or MSW, at least 3 years of relevant experience, and Case Management Certification within 3 years of hire, with NC RN license for BSN candidates.

Qualifications

• BSN or MSW required.
• Minimum of 3 years of relevant experience.
• Case Management Certification (ACM, CCM, or ANCC) required within 3 years of hire.
• For BSN-prepared candidates: NC RN license (or compact) required.

Responsibilities

• Assess patients for case management needs and plan discharge.
• Develop and evaluate individualized discharge plans for timely transitions.
• Coordinate services such as Home Health, DME, transportation, SNF, and rehab referrals.
• Maintain effective communication with the care team and families.
• Maintain timely documentation and ensure regulatory compliance.
• Participate in QAPI activities and provide after-hours coverage as needed.
