Job summary

Duke Health in Durham, NC is hiring a Case Manager to coordinate discharge planning and care transitions across the continuum. You will work with patients, families and interdisciplinary teams to ensure timely assessments, planning, and implementation of safe, cost-effective care.

Requires a BSN or MSW and at least 3 years of relevant experience. Certification in Case Management within 3 years of hire is required; NC RN license (or compact) for BSN holders. Relocation assistance may be available.

Qualifications

• BSN or MSW required.
• Minimum of 3 years of relevant case management experience.
• Case management certification required within 3 years of hire.
• For BSN-prepared candidates, NC RN license (or compact).

Responsibilities

• Assess assigned patients for case management needs and discharge planning.
• Develop and evaluate individualized discharge plans and transitions.
• Coordinate services such as Home Health, DME, transportation, SNF, and rehab referrals.
• Maintain effective communication with the health care team about needs and orders.
• Provide weekend/holiday/after-hours coverage as needed.
