Job summary

Duke University Health System in Durham, NC is seeking a dedicated Case Manager to coordinate discharge planning and care transitions across the continuum of care. You will work with the health team to ensure optimal use of resources and timely planning for post-acute needs.

Qualified candidates have a BSN or MSW, 3 years of relevant experience, and must obtain Case Management Certification within 3 years of hire; current RN licensure in NC is required if BSN holder.

Qualifications

• BSN or MSW required.
• 3 years of relevant experience.
• Case Management Certification (ACM, CCM or ANCC) within 3 years of hire.
• If BSN, current RN licensure in NC or compact state is required.

Responsibilities

• Manage a designated caseload to coordinate timely assessment, planning, implementation and evaluation of discharge plans and care transitions.
• Coordinate with health system resources to arrange Homecare, Durable Medical Equipment, transportation, and post-acute placements.
• Maintain knowledge of benefits, reimbursement guidelines, and regulatory requirements.
• Provide education and guidance to providers, patients and families as needed.
• Document assessments and discharge plans, and support quality improvement activities.
