Job summary

Qualifications

Responsibilities

• Assess patients for case management needs, discharged planning and education.
• Develop and evaluate individualized discharge plans for timely transitions.
• Coordinate Home Health, DME, transportation, SNF placement, and rehab referrals.
• Maintain timely, accurate documentation and ensure regulatory compliance.
• Provide weekend/holiday/after-hours coverage as needed.
