Qualifications

• Requires Case Management Certification (ACM, CCM or ANCC) within 3 years of hire.

Responsibilities

• Coordinate and complete timely discharge planning and care transitions.
• Coordinate case management needs, including care coordination and education.
• Communicate with health care team about assessments and discharge needs.
• Maintain documentation and records per guidelines.
