Job summary

Duke Regional Hospital in Durham, NC is seeking a Part-time Case Manager to manage a caseload, coordinate discharge planning, and ensure smooth transitions across the care continuum. You will collaborate with interdisciplinary teams, patients, families, and community partners to optimize outcomes and resource use while adhering to CMS and regulatory requirements.

BSN or MSW is required with at least 3 years of experience; Case Management Certification is expected within 3 years of hire.

Qualifications

• BSN or MSW degree required.
• Minimum of 3 years of relevant experience in case management.
• Case Management Certification (ACM, CCM, or ANCC) required within 3 years of hire.
• For BSN-prepared candidates: must hold a current RN license (or compact) in NC.

Responsibilities

• Assess patients for case management needs including discharge planning and care transitions.
• Develop, implement, and evaluate individualized discharge plans.
• Coordinate services such as Home Health, DME, transportation, and SNF placement.
• Monitor daily census and ensure timely evaluation of case management needs.
• Maintain timely, accurate documentation of assessments and interventions.
• Provide weekend, holiday, or after-hours coverage as assigned.
