Job summary

Qualifications

• Minimum 1 year of experience in home health or hospital case management.
• Active licensure as RN/LPN/SW/PT in the practicing state.
• Valid driver's license, reliable transportation and current auto insurance.
• Knowledge of Medicare/insurance coverage guidelines.

Responsibilities

• Achieve monthly production and MC admission targets; manage sales expenses.
• Implement strategies to increase market share within assigned facilities.
• Evaluate patients and physician orders for home care eligibility.
• Conduct face-to-face transitions to educate patients and identify the PCP.
• Coordinate transfers and ancillary services (DME, infusion); educate on care plans.
• Educate patients on post-discharge care, meds, and pharmacy use.
• Serve as liaison between the agency, referrals, and providers.
• Provide follow-up and maintain confidentiality per policies.
• Perform sales administration duties and maintain timely communication.
