Job summary

ProHealth Group | Home Health & Hospice is seeking a Care Transition Coordinator to act as the primary liaison with referral sources and educate customers about our services. This role focuses on developing community relations, obtaining patient referrals, and coordinating with patients, families, physicians, and discharge planners.

The ideal candidate has at least 1 year of sales experience; nursing experience can substitute. Requires Alabama driver’s license and reliable transportation.

Qualifications

• Associates degree in Business, Nursing, Marketing, or Healthcare related field, or 5+ years of B2B/industry selling experience.
• Bachelor's degree in Business, Nursing, Marketing or Healthcare related Field preferred.
• One year of sales experience; one year of nursing experience may substitute for sales.
• Two to five years of sales experience; home health or hospice experience preferred.
• Current Alabama Driver’s License.
• Current Automobile Liability Insurance.

Responsibilities

• Care Transition Coordinators serve as the primary liaison with referral sources and educate on company programs and services.
• Develop, maintain and service community relations within the assigned territory.
• Obtain patient referrals and coordinate between patients, caregivers, physicians and discharge planners.
• Manage territory and track referrals, performance, and sales activities.
• Travel within assigned territory as needed.
