Job summary

ProHealth Group | Home Health & Hospice seeks a Care Transition Coordinator to serve as the primary liaison between the company and referral sources, developing and servicing community relations. You will educate patients, caregivers, physicians and discharge planners about the company's programs and services, and drive patient referrals within the assigned territory.

Qualifications

• Associates degree required or 5+ years of B2B or industry selling experience.
• Bachelor’s degree in Business, Nursing, Marketing or Healthcare field preferred.
• Formal sales training also preferred.

Responsibilities

• Manage sales performance and territory budgets on a regular basis.
• Develop weekly/monthly call plans and optimize territory productivity.
• Identify referral sources and expand opportunities within the territory.
• Maintain accurate records of referrals and activity for sales meetings.
• Meet or exceed quotas and ensure timely follow-up on referrals.
