Job summary

Amedisys is hiring a care transitions coordinator in Birmingham. The role requires an RN or LPN license and outside sales experience in healthcare. Travel 50%+ within territory to educate providers, build referral sources, and coordinate patient admissions for home health and hospice services.

You will develop relationships with hospitals and clinics, manage CRM records, and strive to meet admissions and payer-mix goals, all while supporting patients in healing at home.

Qualifications

• RN or LPN clinical license required (state-specific).
• At least 1 year in healthcare business development/outreach or similar role.
• Reliable transportation with valid driver’s license and insurance.

Responsibilities

• Educate healthcare professionals, patients, and families about home care services.
• Develop and maintain relationships with key referral sources (hospitals, nursing homes, physician offices) to generate leads.
• Coordinate patient admissions with the care team, securing required documentation for referral-to-admission.
• Conduct on-site visits to assess patient needs and identify barriers to care.
• Maintain accurate records of patient interactions and referrals in CRM.
• Achieve admissions, product, and payer-mix goals; meet short and long-term target accounts.
• Document daily sales calls in CRM with pre/post-call notes.
