Ascension at Home together with Compassus in Nashville seeks a Care Transition Associate to provide non-clinical administrative and care coordination support for home health and hospice referrals, discharge planning, and start-of-care coordination.

You will liaise with hospital teams, Care Partners, and post-acute providers, manage documentation, educate patients and families, and help ensure timely and efficient care transitions across the care continuum.

Formación

• Associate’s degree or equivalent education and experience is required.
• One year of experience with patients and families and healthcare customer service is required.
• Two or more years of healthcare experience and related coursework are preferred.

Responsabilidades

• Serve as non-clinical liaison between inpatient and outpatient settings for home health and hospice referrals and discharge planning.
• Coordinate referral and admission activities, collect and verify patient information, and assist with scheduling starts of care.
• Gather information to support clinical assessment for home health/hospice; coordinate services per orders and patient needs.
• Communicate with physicians, hospital discharge planners, and referral sources to verify orders and address barriers to timely discharge.
• Provide administrative support: EMR documentation, chart organization, referral/reimbursement coordination, distribute information.
• Educate patients and families about home health/hospice services and coordinate equipment delivery with vendors.
• Maintain knowledge of community resources, payer criteria, and care contracts to support coordination.
• Build positive relationships with hospital teams and post-acute providers; participate in outreach and training as assigned.
• Monitor care arrangements, follow up on outstanding needs, and escalate clinical concerns when needed.
