Job summary

Ascension at Home together with Compassus is seeking a Care Transition Associate to provide non-clinical administrative and care coordination support for patients needing home health and hospice services. You will assist with referrals, discharge planning, and scheduling starts of care, while maintaining HIPAA compliance and strong customer service.

Qualifications

• Associate degree or equivalent education and experience.
• One year of experience with patients and customer service, preferably in healthcare.
• Proficiency with Microsoft Word/Excel; Epic or EMR experience preferred.

Responsibilities

• Serve as non-clinical liaison between inpatient and outpatient settings for home health and hospice referrals and discharge arrangements.
• Coordinate referral and admission activities, collect/verify demographics, insurance and orders, and assist with scheduling starts of care.
• Organize information to support clinical assessment of patient suitability for home health and hospice services.
• Collaborate with clinical teams to coordinate services and equipment per orders and patient needs.
• Communicate with physicians, discharge planners, and referral sources to verify orders and address barriers to discharge.
• Provide administrative support including EMR documentation, chart organization, and reimbursement coordination.
• Educate patients and families about home health and hospice services; coordinate equipment delivery with vendors.
• Maintain knowledge of community resources, payer criteria, and contracts to support care coordination.
• Build positive relationships with hospital teams, referral sources, and post-acute providers.
• Escalate clinical concerns to leadership as needed and comply with HIPAA and other regulations.
