Job summary

Duke Health Lake Norman Hospital in Mooresville, NC seeks a discharge planning case manager to coordinate assessment, planning, and care transitions across the continuum of care, ensuring optimal use of resources and CMS compliance. Responsibilities include monitoring daily census, coordinating Homecare and rehab placements, documenting findings, and participating in QAPI.

Requires BSN or MSW, 3 years' experience, and Case Management Certification within 3 years; a commitment bonus of $7,500 or

Qualifications

• BSN or MSW required.
• 3 years of relevant experience.
• Requires Case Management Certification (ACM, CCM or ANCC) within 3 years of hire. BSN required and must have current or compact RN licensure in NC. or MSW(if no BSN or compact RN licensure in NC)

Responsibilities

• Coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care.
• Maintain effective communication with health care team members related to discharge planning needs and provider orders.
• Arrange Homecare, Durable Medical Equipment, Transportation, Skilled Nursing or Acute Rehab Facility placement; coordination with payers and approvals.
• Maintain timely documentation of assessment findings, discharge arrangements and actions.
• Participate in Quality Assurance/Performance Improvement (QAPI) activities.
