Job summary

Duke Health Lake Norman Hospital in Mooresville, NC, is seeking a Discharge Planning Coordinator to coordinate assessments, planning, and care transitions for a designated caseload across the continuum of care.

You will collaborate with the care team to arrange Homecare, DME, transportation, and facility placements while ensuring compliance with CMS guidelines. A BSN or MSW is required, with 3 years of experience and Case Management Certification within 3 years of hire.

Qualifications

• BSN or MSW required.
• 3 years of relevant experience.
• Requires Case Management Certification within 3 years of hire.
• RN licensure in NC (or MSW if no BSN or NC licensure).

Responsibilities

• Coordinate discharge planning and care transitions across the continuum of care.
• Maintain effective communication with health care team and arrange Homecare, DME, transportation, and facility placements as needed.
• Document assessment findings and actions; participate in QAPI activities and quality improvement initiatives.
• Develop and maintain positive working relationships with customers internal and external to Duke Health System.
