Job summary

Required BSN or MSW, NC RN licensure (or valid compact) and Case Management Certification within 3 years. Three years of relevant experience is expected. Relocation assistance may be provided based on eligibility.

Qualifications

• 3 years of relevant experience
• BSN or MSW required
• Case Management Certification (ACM, CCM or ANCC) within 3 years of hire

Responsibilities

• Coordinate and complete discharge planning and care transitions across the continuum of care
• Monitor daily census and assign care coordination/transition activities
• Provide education and guidance to providers, patients and families on discharge planning and benefits
