Job summary

BrightSpring Health Services, via its Adoration Health affiliate, seeks a Care Transition Coordinator in Maryland to support seamless transitions from healthcare facilities to home health or hospice care.

You will evaluate eligibility, coordinate care plans, and manage patient education while liaising with referral sources, facilities and providers to promote growth and compliance. Strong communication and documentation are essential to align with agency protocols.

Qualifications

• Minimum of 1 year in home health or hospital-based case management.
• Preferred: 1–3 years in medical marketing or healthcare business development.
• Active licensure in RN/LPN/SW/PT in state of practice is required.
• RT certification or clinical program knowledge preferred.

Responsibilities

• Achieve monthly MC admission targets for assigned locations and manage expenses for financial stewardship.
• Implement strategies to increase market share within facilities.
• Evaluate patient eligibility for home care per Right of Choice guidelines.
• Conduct face-to-face transitions and educate patients and physicians.
