Job summary

Terros Health is seeking a Discharge Planner to coordinate care between inpatient units and outpatient services at our Phoenix site. You will facilitate transitions for Level I inpatient clients to Terros Health’s outpatient programs, ensuring continuation of care and minimizing readmission risks.

The role requires strong communication, collaboration with providers, and adherence to privacy and policy. Eligible candidates typically hold a High School Diploma or GED with health experience, or a

Qualifications

• High School Diploma or GED.
• Combination of behavioral health education and work experience totaling 4+ years; or a bachelor’s degree with 1 year of behavioral health experience; or a bachelor’s degree in a behavioral health field.

Responsibilities

• Discharge Planner activities in inpatient and outpatient settings.
• Connect individuals with ongoing outpatient treatment after discharge.
• Coordinate with hospital staff, outpatient team, families and care management to ensure smooth transition to community services.
• Maintain confidentiality of client records and comply with policy.
