Job summary

Terros Health is seeking a Discharge Planner to join our interdisciplinary clinical team in Phoenix. You will coordinate discharge planning for individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment at our Mitchell clinic. The role is full-time, 40 hours/week, Monday–Friday, located at 40 E.

Qualifications

• GED/High school diploma with a minimum of 4 years behavioral health employment experience
• Associates degree in behavioral health + 2 years behavioral health employment experience
• Bachelor's degree (not in behavioral health) + 1 year behavioral health experience
• Bachelor's degree in behavioral health
• Master's degree or higher in behavioral health
• Microsoft Word, Excel, Outlook, electronic medical record experience preferred
• Valid AZ Driver License and 4+ years driving experience
• Arizona Fingerprint Clearance Card requirements

Responsibilities

• Review clinical information, identify resources, and help coordinate discharge into the community with follow-up.
• Coordinate inpatient and outpatient activities, including visits, meetings, and service planning.
• Connect Inpatient Level I clients with ongoing outpatient TerrosHealth treatment after discharge.
• Collaborate with hospital staff, outpatient team, families, PCPs, and health plans for smooth transitions.
• Participate in individualized service planning and execute discharge plans.
• Offer coordination of services addressing recovery goals and reduce readmission risk.
