Community Bridges, Inc. (CBI) is an integrated behavioral health agency offering residential, outpatient, inpatient, medical home, medication‑assisted treatment, and crisis services across Arizona. The Transition Coordinator supports patients at each level of care to ensure a seamless discharge and ongoing treatment plan.
Transition Coordinators are responsible for developing individualized discharge plans, scheduling follow‑up appointments within seven days of discharge, coordinating authorizations through Utilization Management, and completing initial assessments and treatment plans. They provide therapeutic services—including counseling, group work, and discharge planning—to patients and their support systems.
