Job summary

Terros Health is seeking a Discharge Planner to serve as the liaison between inpatient settings and the assigned clinician at the Outpatient site. The role emphasizes coordinating transitions, identifying community resources, and monitoring follow-up with psychiatric providers.

The position requires a High School Diploma or GED, or a bachelor’s degree in behavioral health with relevant experience. Candidates must have a valid AZ driver’s license and pass fingerprint clearance.

Qualifications

• High School Diploma or GED.
• Bachelor’s degree in behavioral health field OR a bachelor’s in any field with relevant BI health experience.
• Proficiency with Microsoft Word, Excel, Outlook; EMR/NextGen experience preferred.
• Valid AZ Driver License; 21+ with 4 years driving experience; driving policy compliance.
• Must pass fingerprint clearance and pre-employment screening.
• Strong interpersonal and communication skills; ability to maintain confidentiality.

Responsibilities

• Discharge Planner activities in inpatient and outpatient settings.
• Connect inpatients with ongoing TERROS outpatient treatment on discharge.
• Coordinate with hospital staff, providers, families and RBHA care management for smooth transitions.
• Maintain client confidentiality and adhere to all pre-employment and policy requirements.
