Qualifications

Responsibilities

• Discharge planning coordination between hospital services and community.
• Manage discharge flow from inpatient to safe transition home or next level of care.
• Assist with locating services, scheduling follow-ups, and transportation arrangements.
• Initiate follow up calls to external referrals and manage care point alerts.
• Track discharge data, compile reports, and audit discharge paperwork for accuracy.
• Ensure discharge records are sent to receiving providers.
• Perform other duties as assigned.
