Job summary

AdvantageCare Physicians (ACPNY) is expanding its primary and specialty care network in the New York metropolitan area. The role provides professional nursing care to patients, coordinates care, and serves as a key liaison among patients, staff and physicians in the Advanced Care Model.

You will supervise the specialty care team, assist with staff scheduling, mentoring, and ongoing education, and participate in huddles and quality initiatives across the practice.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor's degree in nursing preferred.
• Current unencumbered New York State Registered Nurse license.
• BLS
• ACLS required for GI and Cardiology

Responsibilities

• Provide professional nursing care to patients and support patient satisfaction, safety, and quality of care.
• Serve as liaison between patients, staff and physicians and support the Advanced Care Model.
• Organize clinical supervision to the specialty patient care team and delegate tasks as appropriate.
• Support new hire orientation, staff training, and performance evaluations.
