Qualifications

Responsibilities

• Coordinate daily schedules for Home Health Aides (HHAs).
• Assign caregivers to patient cases and arrange replacements when needed.
• Monitor caregiver attendance, coverage, and missed visits.
• Communicate scheduling changes with caregivers and contracted facilities.
• Resolve call-outs, scheduling conflicts, and caregiver concerns.
• Maintain continuity of care whenever possible.
• Document staffing changes, incidents, and patient-care issues.
• Maintain accurate staffing and compliance reports.
• Work closely with Nursing, Compliance, Recruitment, and Payroll/Billing.
• Participate in scheduled on-call coverage as required.
