Job summary

Caring for Others, a family-owned in-home care provider in Oregon, seeks a care coordination professional to serve as the central hub for Clients, families, Caregivers, and partners. You will coordinate care services, develop individualized plans, and ensure compliant, high-quality support to Clients in their homes and by phone.

The role emphasizes timely assessments, authorization management, and staffing to maintain consistent coverage.

Qualifications

• Experience in home care, healthcare coordination, or client-facing service role preferred.
• Highly organized with strong attention to detail and accurate record-keeping practices.

• Committed to compliance, HIPAA standards, and accurate documentation.

Responsibilities

• Deliver 5-star service on every call as the hub for Clients, families, Caregivers, Case Managers, and referral Partners.
• Coordinate care services for timely, consistent delivery to Clients in home or by phone.
• Develop individualized service plans and conduct initial and ongoing Client assessments.
• Manage authorizations and marry activities to support billing accuracy.
• Support staffing to minimize overtime and ensure coverage of scheduled shifts.
• Maintain Branch compliance and licensing documentation.
• Lead weekly Caregiver New Hire training and prepare intake materials.
