Job summary

Centene Management Company LLC is seeking a Navigator to develop, assess, and coordinate care management activities for members in Illinois. The role primarily works remote (75%) with travel (25%) to listed counties and requires a Monday–Friday schedule in the Central time zone, including occasional evenings and weekends based on need.

The role involves developing personalized care plans, educating members and caregivers on services, coordinating with care teams, and maintaining member records

Qualifications

• Requires a Bachelor’s degree and 2 - 4 years of related experience.
• Graduate from an Accredited School of Nursing if holding clinical licensure.
• Equivalent experience acquired through related duties and responsibilities.

Responsibilities

• Evaluates member needs, barriers to care, resources available, and recommends the best care plan.
• Develops or contributes to personalized care plans and identifies providers or community resources as needed.
• Provides psychosocial and resource support to members and caregivers to access local services.
• Coordinates between member/family and care provider team to ensure timely access to services.
• Monitors progress towards care plan goals and collaborates with healthcare providers for revisions when needed.
• Collects, documents, and maintains all member information to ensure regulatory compliance.
• May perform on-site visits and provide education to care managers and members.
