Job summary

Rezilient Health in Missouri is seeking a Care Navigator to guide patients through the entire healthcare experience, coordinating scheduling, chart prep, referrals, medical records, and follow-up. You will work closely with providers, medics, and the Care Management team to keep patient care continuous and informed, removing barriers to timely care.

Qualifications

• At least 3 years in healthcare, direct patient support.
• Experience scheduling multi-provider appointments.
• Able to work in fast-paced, changing environments.
• Experience documenting in electronic medical records.
• Experience requesting and tracking external medical records.
• Knowledge of HIPAA and healthcare regulations.
• Strong communication and interpersonal skills.
• Excellent organizational skills and attention to detail.
• Self-motivated with ability to manage independent tasks.
• Based in Missouri, Illinois, Indiana, or Minnesota.

Responsibilities

• Guide patients through the Rezilient healthcare experience.
• Schedule appointments and prep charts in required systems.
• Request medical records from outside providers ahead of appointments.
• Advocate for patients and remove barriers to timely care.
• Provide concierge support through fragmented healthcare systems.
• Coordinate referrals to specialty care, labs, imaging, etc.
• Assign tasks and document patient journey.
• Respond to patient portal messages and calls with professionalism.
• Collaborate with providers, medics, and care teams.
• Escalate issues to appropriate personnel promptly.
• Maintain HIPAA-compliant documentation.
• Attend meetings and participate in training as needed.
