Rezilient Health is hiring a Care Navigator to guide patients through scheduling, chart preparation, referrals, and records management. You will collaborate with providers and care teams to ensure timely, informed patient experiences and smooth communication across platforms.

The role emphasizes compassionate support, proactive problem-solving, and strong documentation as part of a hybrid healthcare delivery model in a fast-paced setting.

Qualifications

• Minimum 3 years in a healthcare setting delivering patient service.
• Experience scheduling multi-provider appointments and referrals.
• Knowledge of HIPAA and healthcare regulations.
• Excellent written and verbal communication; ability to build rapport.

Responsibilities

• Guide patients through the Rezilient healthcare experience and coordinate each step of the patient journey.
• Schedule patient appointments, prep charts, and enter information in required systems on time.
• Request and collect medical records from outside providers so charts are complete before visits.
• Advocate for patients by tracking needs and communicating with providers and clinics.
• Provide concierge support to help patients navigate fragmented healthcare and insurance.
• Coordinate referrals to specialty care, imaging, labs, and community resources.
