Job summary

MultiCare Tacoma General Hospital seeks a Care Coordinator Associate to support hospital transitions within an interdisciplinary team. You will help ensure smooth throughput, coordinate scheduling, and assist with insurance authorizations while using multiple data platforms.

The role emphasizes patient-centered care, collaboration, and strong customer service. A health-related background and experience in healthcare settings are preferred, with a focus on transitions of care.

Qualifications

• Post-secondary coursework, preferably in a health related field.
• Bachelor’s degree highly desirable.
• Minimum of 2 years experience in a health care setting, preferably as a HUC or in a clinical role such as MA, CNA or EMT.
• Experience in utilization management, community resources, and/or financial and quality issues affecting patient care desirable.

Responsibilities

• Assist in cross continuum hospital transitions as part of an interdisciplinary care team.
• Support scheduling, insurance authorizations and care coordination processes.
• Leverage multiple data platforms to ensure smooth throughput and patient flow.
