Job summary

Kindred Hospital Northland is seeking a care coordination professional to ensure seamless transitions and optimal resource use across the continuum of care. You will collaborate with the ICT team, clinicians and payors to support discharge planning and patient outcomes.

Responsibilities include coordinating clinical and psychosocial activities, referring high-risk patients for additional support, and advocating for patients while maintaining compliance with regulations.

Qualifications

• Graduate of an accredited program required: LPN/LVN or RN.
• Master of Social Work with licensure; or Bachelor of Social Work with licensure.

Responsibilities

• Coordinate clinical and psychosocial activities with the Interdisciplinary Team and Physicians.
• Assist with care coordination and efficient care facilitation.
• Refer high-risk patients for additional support.
• Serve as a patient advocate.
• Participate in interdisciplinary patient care rounds and discussions with the care team.
