The Elizabeth Layton Center in Paola, KS is seeking a Service Navigator to join our Operations team. You will collect, manage, and report data related to CCBHC outcomes, including NOMs, and collaborate with clients, families, and providers to ensure a smooth intake and care experience.

The ideal candidate brings strong data entry skills, attention to confidentiality, and the ability to work cross-functionally.

Competenze

• High School diploma or equivalent required.
• Minimum 3 years clerical or general office experience.
• Must be able to pass required background checks.

Mansioni

• Collects and reports National Outcome Measures (NOMS) and other data.
• Enters NOMS data into the SPARS system in a timely manner.
• Completes patient intake documentation and health measurements as required.
• Maintains confidentiality of all employee and client information.
• Coordinates with internal and external providers to improve outcomes.
• Cross-trains in intake duties to provide back-up during the day.
• Ensures accurate data entry into spreadsheets tracking information.
