Job summary

Switchboard Health, Inc. seeks a Care Navigator to support patients seeking specialty care and to coordinate referrals. This part-time, 1099 contract role offers flexible hours (about 15–25 hours/week) and pay of $20–$30 per hour.

You will engage patients by phone, portals, text, and email; schedule appointments; and maintain PHI privacy in a HIPAA-compliant environment. Prior experience in healthcare referrals and EMR/CRM tools is preferred.

Qualifications

• Experience in healthcare referrals or patient access
• Ability to use EMR/CRM tools across platforms
• Excellent verbal communication and customer-service mindset
• Familiarity with PHI and HIPAA/privacy standards

Responsibilities

• Engage with patients via phone, portals, text, and email to assess care needs.
• Provide information about available specialty services and resources.
• Coordinate and schedule appointments with specialty providers.
• Maintain accurate, timely documentation in secure systems.
• Outreach to specialty offices to determine appointment availability for new patients.
• Support process improvements and patient experience initiatives.
