Switchboard Health, Inc. seeks a Care Navigator to support patients seeking specialty care and to coordinate referrals. This part-time, 1099 contract role offers flexible hours (about 15–25 hours/week) and pay of $20–$30 per hour.
You will engage patients by phone, portals, text, and email; schedule appointments; and maintain PHI privacy in a HIPAA-compliant environment. Prior experience in healthcare referrals and EMR/CRM tools is preferred.
The Care Navigator supports Switchboard Health clients in two primary ways: Directly assisting patients seeking specialty care, and Conducting outreach to clinician offices to determine appointment availability for new patients. This role helps patients connect with the right specialty providers (e.g., behavioral health, dermatology, gastroenterology, cardiology, and others) by providing resources, coordinating referrals, and scheduling appointments.
