Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to oversee coordination of care for ROWI clients, including orientation, transportation, doctor visits, and scheduling. This role supports program delivery and ensures high standards of care within a collaborative team.

The position requires strong communication, organization, and interpersonal skills, plus a minimum of two years of driving experience and experience with youth in a therapeutic setting.

Qualifications

• Minimum of a high school diploma; Bachelor’s degree preferred.
• Minimum of two years driving experience.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.

Responsibilities

• Coordinate individualized service including orientation, transportation, doctor visits, and schedule.
• Ensure programs and services are implemented to high standards; clients receive high level of care.
• Supervise clients and ensure attendance at designated groups.
• Facilitate groups and activities as designated by Facility Manager.
• Conduct client orientation and relevant assessments.
• Comply with Joint Commission Standards and document shift reports.
• Coordinate transportation and drive clients as needed; document in EMR.
• Administer and submit EMR documentation for urine analysis.
• Maintain facility cleanliness and coordinate client meals.
• Maintain confidentiality and professional boundaries; attend staff meetings.
